About the Role
At Help for Heroes, our Case Managers play a vital role in supporting veterans and their families to live well after service. Acting as a key point of contact, you will help individuals access the services, resources, and opportunities they need to improve their wellbeing and lead fulfilling lives.
As a Case Manager, you will:
Conduct comprehensive initial assessments to identify each veteran’s unique needs and challenges.
Develop and implement personalised support plans that reflect individual goals and promote positive outcomes.
Monitor and review progress regularly, adapting plans as circumstances evolve.
Advocate on behalf of veterans to ensure they receive the benefits, services, and entitlements they are eligible for.
Provide clear, practical information on navigating the support systems available, empowering veterans to make informed decisions.
Support veterans to build confidence, develop life skills, and increase independence through tailored guidance and encouragement.
Maintain a visible presence within the community, including conducting home visits and offering hands-on support.
Build strong partnerships with local organisations and services to create effective support networks for veterans and their families.

Salary: Up to £33,000
Location: Homebased/Regional - North Yorkshire, West Yorkshire, County Durham, Cumbria and Northumberland.
Regular travel within the above regions is expected. Applicants will need to confirm that they are situated in one of the above regions on application.
Hours: 35 per week, Monday - Friday
